I can give medical care smoothly when you have the letter of introduction of the medical institution or prescribed medicine.

- My number card (when you do not have it, it may become the request for own expense)
- Identification of other recipients (e.g.,: identifications of identification of identification of elderly person recipient, specific disease medical care recipient, other medical care)

In the case of a reception desk, please let the documents making desired person know.
